  5. Were you involved in the development of Safe Work Method Statements?
  6. What would you do in the event of an emergency?
  7. Has there been a safety inspection on the site this week or last week?
  8. Has your Supervisor talked to you about safety this week or last week?
  9. What safety issue or topic was discussed with your Supervisor this week or last week?
  10. How would you rate the contractors commitment to safety on this job compared to other jobs ou have had (better/similar/worse)?
  11. Are you encouraged to report safety hazards?
  12. If you had a safety concern about something you saw on the job, who would you report it to?
  13. If you reported something you thought was unsafe, do you think it would be taken seriously?
  14. Have you seen the company OHS & R Policy?
  15. Have you seen the Minutes of the most recent OHS Committee meeting?
  16. Do you have a 'job-start' or 'tool box' meetings to plan the work for each day?
  17. Are safety issues discussed at this meeting?
  18. When did you receive your site induction training?
  19. What training have you had while working on this site?
  20. Can you show me your site induction card, relevant licences, etc?
